Pleiospilos nelii is a fleshy perennial plant that can reach a height of 5-8 cm (2-3 inches) and a diameter of about 10 cm (4 inches). This very short-stemmed, almost stemless plant has one or more opposite, almost hemispherical, gray-green or brownish leaves. There are many small dark spots on the surface of the leaves. Every year, a new pair of leaves is produced to replace an old pair. This plant is very similar to a small cracked rock that may have evolved as a defense against herbivores. Superficially chamomile-like flowers are yellow-orange, 6-7.5 cm (2-3 inches) wide, and emerge from the apical bud of the plant. The flowers are large according to the size of the plant. Each flower opens in the afternoon and closes at sunset during a flowering period of several days.
